1 : Copy the CM 12.1 Rom on your SD Card.
2 : Boot into Custom Recovery (TWRP)
3 : Make full wipe including "system" (data, dalvik, system, cache)
4 : Now go back and click install zip
5 : Flash the custom rom
6 : Reboot (the first boot can take up to 4-6 minutes)
